Who eligible to apply for Registration of a Death

The deceased is a Citizen of Sri Lanka

The death had occurred in Singapore or in Brunei Darussalam

The Applicant should be the nearest relative of the deceased or a relative of the deceased who resides in the country, where the death occurred or the main occupant of the house where the death occurred or the doctor in charge of the hospital/ public institution where the death occurred if there are no such person who takes action to dispose of the deceased by burial, cremation or other means.

The Application for registration of death should be made to the High Commission within three months of the death. (Under “Section 30”)

  • When an Application is made after 03 months and within one year, reason for delay should be submitted in writing to the High Commission. (Under “Section 36”)
  • When an Application is made after one year, an explanation addressed to the Registrar General should be forwarded, before such a Death is registered at the High Commission.

Required Documents

(Original documents should be submitted with TWO set of photocopies)

  • Declaration Form filled by the declarant, documents to prove the identification of the declarant. Submit a document to prove the deceased is a Citizen of Sri Lanka. (e.g. Valid Sri Lanka Passport together with current VISA, Sri Lanka Dual Citizenship Certificate, etc)
  • Death Certificate issued by the relevant country /Hospital records of deceased. (English translation of the Death Certificate attested by the Foreign Ministry of the issued Country must attached if the document issued in a language other than Sinhala, Tamil or English)
  • If the cause of the death is not included in Death certificate issued by the relevant country, it should be proved by supporting documents.
  • Birth Certificate of the deceased
  • Cargo receipts will be needed, if the death is been registered in Sri Lanka
  • Documents to prove the identification of the declarant

Dispatch of the Human Remains to Sri Lanka or Bury or Cremate in Singapore

  • If the relatives intend to dispatch the human remains of Sri Lankans deceased to Sri Lanka for the burial/cremation, it is required to obtain prior approval from the High Commission.
  • If the relatives intend to bury/cremate the human remains of Sri Lankans deceased in Singapore, they are also required to obtain a prior approval from the High Commission.
Therefore, whenever the death of a Sri Lankan occurs, the relatives are instructed to contact the High Commission, giving following details in writing:

Requests for approvals for repatriation of Human remains should be submitted by the relevant airline/Sri Lankan Airline Cargo to the Airport Health Office with following documents.

  • Death Certificate
  • COVID-19 Test Report
  • Coroners certificate/Police/Forensic Report If the (death was not due to natural reasons)
  • Embalming Certificate and Sealing Certificate
  • High Commission no Objection Letter
  • Proof of identification of deceased (Cancelled Passport)
  • National Environmental Agency Certificate
  • Consignee contact details
  • Contact details of the immediate relatives of deceased living in Singapore/Sri Lanka (preferably two names with telephone numbers)
  • Upon receipt of above information, the High Commission will inform the death to the Consular Division of the Ministry of Foreign Affairs of Sri Lanka.
  • Thereafter, the Consular Division of the Ministry will contact (as per information provided by the informant) one of the relatives in Sri Lanka and obtain an affidavit from him/her and send it to this High Commission along with their instructions.
  • Upon receipt of the instructions from the Consular Division, the relatives /friends of the deceased in Singapore will be informed by this High Commission to proceed with the arrangements.
  • The immediate family members should bear all cost pertaining to the burial/cremation of the human remains in Singapore or for the dispatch of human remains to Sri Lanka.
  • Please note that the original passport of the deceased needs to be immediately handed over by the relatives/informants to this High Commission for cancellation, along with the no objection letter issued by the High Commission for the repatriation of the human remains to Sri Lanka.
